Nearly 50% of individuals entering incarceration have a substance use disorder, and research documents an increased risk of overdose post-release. Many individuals leaving jails and prisons find it challenging to access continued treatment, medication, housing, employment, and transportation during the immediate post-release period. Corrections agencies can improve outcomes for those reentering the community by strengthening in-reach care coordination and discharge planning, employing community navigators and peer support specialists to provide recovery support, and improving information sharing between correctional health care providers and community providers.
