This brief discusses the use of specialized caseloads for individuals with substance use disorders and mental illness. It describes five key practices that support successful implementation. These practices include developing a strong program infrastructure, defining the target population and utilizing assessments to determine needs, developing specialized case plans, creating linkages to care with collaborative case management, and tracking program outcomes to demonstrate successes.
